New Patients

We are currently accepting new patients.

If you would like to become a patient of our office please fill out our Medical History, Consent for Electronic Communication, HIPAA, and Notice of Out of Network Provider forms before your initial appointment.

Please note that these forms are lengthy and should be filled out on a desktop computer or printed out and filled in manually.

Send electronically to donahue@chevychasedds.com or print it out and bring it with you to your appointment. Filling out the form before your appointment will ensure that you will be seen promptly.

Although we are a fee-for-service office and do not participate in insurance plans we will file insurance claims on your behalf. Please bring your dental insurance card so that we may help you process your insurance claim.
